What is the thyroid?
It's a small, butterfly-shaped gland sitting at the front of your neck, just below your Adam's apple. It's tiny - but it quietly helps run an enormous amount of your body. Here it is, in place.

Where it sits
The thyroid wraps around the front of your windpipe (the trachea), in the lower part of your neck. It has two halves - the left and right lobes - joined in the middle by a thin bridge called the isthmus. That two-lobes-and-a-bridge shape is why people call it butterfly-shaped.
What it actually does
The thyroid makes hormones - mainly T4 and T3 - that act a bit like your body's accelerator. They help set how fast nearly every cell works: your energy, temperature, heart rate, weight, digestion, mood, even how clearly you think. When it makes too much or too little, you feel it almost everywhere - which is exactly why thyroid symptoms can be so wide-ranging and so easy to dismiss.
The control loop
How it's controlled: your brain is the manager
Your thyroid doesn't decide things on its own. A pea-sized gland deep in your brain - the pituitary - keeps watch and tells it what to do. Here it is, glowing, tucked under the brain.

The pituitary checks how much thyroid hormone is in your blood, then sends out a signal called TSH (thyroid-stimulating hormone) - basically an instruction: "make more" or "ease off." The thyroid responds, the levels change, the pituitary adjusts the signal again. It's a constant feedback loop, like a thermostat.
This is the clever part: it means TSH is often the most useful number on your blood test. If your thyroid is under-active, the pituitary shouts louder and TSH goes up; if it's over-active, the pituitary backs off and TSH goes down. So a TSH result tells your doctor a lot about what your thyroid is actually doing - which is why it's usually the first thing they check.
