Spreadsheet templates

Prefer your own spreadsheet? Download, open in Excel or Google Sheets, and start building a record that's yours to keep - no app, no account, no expiry date.

Your numbers stay on your phone · no accounts, no cookies, nothing to sell you

These are a starting point for anyone who'd rather work in their own spreadsheet. If you'd prefer something interactive and already set up, the lab tracker, medication timing helper, and symptom journal do the same job in the browser - no spreadsheet app needed.

Thyroid lab log

A running record of every blood test so you can spot trends across appointments - and have the numbers ready when you need them.

Columns included

  • Date
  • TSH with lab reference range
  • Free T4 with range
  • Free T3 with range
  • Note

Tip: colour-code rows where a result sat outside the printed range - it makes patterns visible at a glance.

Medication log

Keep a simple diary of every medication and supplement - dose, time, and anything worth noting. Useful when your prescription changes.

Columns included

  • Date
  • Medication name
  • Dose
  • Time taken
  • Note

Tip: log the time precisely - thyroid hormone timing relative to food, coffee, and supplements really matters.

Symptom diary

Rate eight common thyroid symptoms daily on a 1 - 5 scale. Over time the pattern can help you describe what's happening to your clinician in concrete terms.

Columns included

  • Date
  • Energy (1 - 5)
  • Mood (1 - 5)
  • Brain fog (1 - 5)
  • Palpitations (1 - 5)
  • Cold tolerance (1 - 5)
  • Sleep quality (1 - 5)
  • Resting heart rate
  • Note

Tip: 1 = very poor / very symptomatic, 5 = excellent / symptom-free. Keep the scale consistent.

Getting started

1

Download

Hit "Download CSV" on the template you want. It saves straight to your Downloads folder.

2

Open it

Open in Excel, Numbers, LibreOffice Calc, or import into Google Sheets (File → Import).

3

Make it yours

Delete the example rows and start entering your own data. Add columns, colour rows, create charts - it is your file.

No accounts, no cookies, nothing to sell you. Once downloaded, the file lives on your device and we never see anything you put in it. Want a backup? Save a copy to your own cloud storage (Google Drive, iCloud, OneDrive) any time - it is your data.

These tools help you record and make sense of your own information - they don't diagnose, monitor or interpret anything, and they aren't a substitute for medical advice. Where a tool shows a reference range, that is the range from your own lab report, not ours. Everything you enter stays on your device; we never see it. Take what you record to your GP or clinician - only they can tell you what it means for you.
